Insurance
We accept out of network insurance as payment, provided acupuncture is covered by your insurance plan. Once you schedule an appointment you will receive an intake form with a section dedicated to insurance information. Please fill out the form and I will inform you of coverage within 48 hours.
Cancelation Policy
Please be advised that there is a 24 hour cancelation and rescheduling policy. Less than 24 hour notice, you will be charged in full for the missed appointment.
New Patient Intake Form – New patient intake form will be emailed once your first appointment is booked. Please fill it out to the best of your ability.
